process recently undertaken at the ravaged Fukushima Daiichi nuclear power plant in Japan. According to Perrow, if any two of the 1,535 fuel rods held within the reactor touch, it could cause an uncontrollable nuclear reaction that would warrant the evacuation of Japan and the West Coast of the United States.
