I use:
PeerGuardian 2 - premier IP blocker. PeerGuardian 2 integrates support for multiple lists, list editing, automatic updates, and a variety of protocols

But I've heard good things about:
PeerBlock - PeerBlock lets you control who your computer "talks to" on the Internet. By selecting appropriate lists of "known bad" computers
