Let's say I make a video which is basically a slideshow made up of various pictures found online. There are three short clips from two movies and background music is 3 cut/merged/adjusted clips from The Fountain. The whole thing is only about 17 minutes long. Is this allowed? Do I have to first add a "Fair Use" notice at the beginning? Any help would be great.

Yes add Fair Use. To be safe.
Yes it is allowed and add Fair Use to be safe?

Certainly from our perspective, what you are describing is a derivative work, so would be fine as an upload. As for a fair use disclaimer, that's of course entirely up to you.
Some say:

Post it as an EDUCATIONAL video.

Copy & paste this paragraph into the MOREINFO section:

Copyright Disclaimer Under Section 107 of the Copyright Act 1976, allowance is made for "fair use" for purposes such as criticism, comment, news reporting, teaching, scholarship, and research. Fair use is a use permitted by copyright statute that might otherwise be infringing. Non-profit, educational or personal use tips the balance in favor of fair use.

I saved this from somewhere unknown that said it worked. I suspect it may well help a lot as long as there's not a direct complaint from the copyright holder (as opposed to some knob trying to cause trouble).

HTH.
You could also use marionettes and a synthetic Anon voice, thereby avoiding copyright infringement entirely.

Alternative Media turned into Alternative Entertainment through the use of claymation David Icke and Alex Jones mock-ups! Aww yeah.
